Woman with World’s Largest Breasts Fighting Staph Infection

Sheyla Hershey, a Brazilian model with the largest breasts in the world, is experiencing complications following her latest breast surgery.

After more than 30 plastic surgeries, Hershey suffered her first complication when a severe staph infection reached both of her breasts following her most recent breast augmentation procedure in Brazil this past June, according to Fox News in Houston.

The model and mom, who lives in Houston, said her new surgeon told her she should be concerned for her life because the infection could leak into her bloodstream. Hershey has had to give herself infusions of intravenous antibiotics every twelve hours to fight the infection.

While frightening, is this complication common after breast surgery?

Dr. Michael Will, who performs breast augmentation and breast implant repairs, had this to say:

Breast augmentation procedures should be preceded by a comprehensive consultation between the cosmetic surgeon and the patient, where the patient’s concerns and expectations are discussed in relation to the patient’s breast and chest wall anatomy.

An effective evaluation and treatment plan should result in a post-operative outcome that reaches a balance between the patient’s expectations and the surgeon’s surgical and anatomical limitations to yield an esthetic and healthy outcome. When the surgical tissue tension and stress exceed the capacity for normal healing, wound breakdown and infection often develop.

However, infection related to breast augmentation is rare, and when encountered, the implants frequently need to be removed and replaced at a later date.

In 2009, Hershey was a 38KKK and received the Brazilian equivalent of the Guinness Book of World Records award.

Hershey is currently undergoing treatment, according to her website www.sheylahershey.net, which includes updates on her condition.

Mentor’s Breast Augmentation Tool Helps Patients Envision Implants

A Guide to Making Your Own Breast Implant Sizers

If you’re thinking about breast augmentation, the size of your implants is one of your most important decisions. The right size is key to your satisfaction. Your surgeon will help determine what size fits your shape and chest wall anatomy, but you can also make your own breast implant sizers to help you decide.

Wearing the sizers at home can help you figure out which size will make you feel most comfortable and look best under your clothes.

Rice implant sizers are a simple and cheap DIY project that allows you to test out various implant sizes. Here’s what you need to make your own rice implant sizers:

  • Panty hose or knee high hose
  • Measuring cups
  • Dry rice
  • Scissors

To make your breast implant sizers, first cut the legs off of the panty hose halfway from the bottom and stretch each out. Then measure your rice carefully, using the following guidelines:

  • 1 cup is equal to 250 cc
  • ½ cup is equal to 125 cc
  • ¼ cup is equal to 75 cc

After you’ve filled the panty hose, compress the rice into the bottom into a spherical shape. Then tie a knot in the panty hose right above the rice filling, cut off the remaining tail of the panty hose, and you’re done.

Put your rice implant sizers in your sports bra and wear it under your tops to get an idea of how a specific implant size will look and feel. Surgeons Meet for Annual Breast Surgery Workshop

40 cosmetic surgeons recently met for the 4th annual breast surgery workshop at the Ohio cosmetic surgery practice of Dr. Mark Mandell Brown. To attend the workshop, doctors traveled from England, Canada, Taiwan and other areas of the U.S.

The course was sponsored by the American Academy of Cosmetic Surgery, the nation’s largest medical society of cosmetic surgeons. During the 3 day workshop, surgeons discussed cosmetic procedures such as breast augmentation, breast lift, and breast reduction surgery.

Through a combination of lectures, surgery, and anatomy labs, the visiting physicians can improve their skills.  “It’s also a great way to showcase Cincinnati to our visiting doctors” observed Therese Holden, Nursing director.  She added, “we really strive to make this an outstanding educational experience for the participants.” “Our breast course has earned excellent recognition and is one of the most popular breast courses in the Academy,” said Dr Mandell-Brown.

Assessing Breast Asymmetry Before Breast Augmentation

Women with breast asymmetry who are considering a breast augmentation need to be aware that it may accentuate the imperfection, especially when the asymmetry is more pronounced, according to surgeons who participated in a panel on the topic at the recent annual meeting of the American Society for Aesthetic Plastic Surgery.

Dr. Jack Fisher, who served as panel moderator, said, “It is important for surgeons to assess the patient’s breast symmetry before operating to determine whether the surgery is necessary and if it will make the asymmetry more obvious.”

The key to a high patient satisfaction rate is “to fully inform the patient that this kind of augmentation is extremely challenging, but that using a detailed analytic approach, we can achieve a very aesthetically pleasing outcome within the limits set by their preoperative anatomic characteristics,” according to Dr. Laurie Casas, who was on the panel.

In order to increase patient satisfaction, surgeons must create a breast augmentation plan that evaluates all elements that contribute to breast asymmetry, including volume, nipple position and chest wall deformity.

The cosmetic surgeon should also be clear with the patient about the challenges of breast augmentation posed by breast asymmetry, so that patients are fully informed and have realistic expectations about their results. In addition, new technology, such as 4-dimensional imaging, can help patients have reasonable expectations before surgery.

Surgeon Says Hyaluronic Acid Gel Injection Effective for Breast Enhancement

injections for breast enhancement A Swedish doctor says that hyaluronic acid gel injections are effective for breast enhancement.

Per Hedén, MD, PhD, says the injectable is a simple and safe alternative for women who want to enhance their breasts, but aren’t ready to have breast implant surgery.

“Hyaluronic acid gel injection … provides immediate results with minimal discomfort, and in clinical trials, it was associated with high ratings of breast general appearance, shape and firmness,” Dr.  Hedén told Cosmetic Surgery Times.

The hyaluronic acid gel that has been developed for use on the breast, Macrolane, is approved for use in Europe but does not currently have FDA approval in the United States.

Dr. Hedén says there have been no serious complications with Macrolane noted in studies, but hyaluronic acid gel for breast enhancement is new, so there isn’t yet long-term data on the procedure.

Breast Implant Alternative?

While the filler offers a less-invasive alternative to implants, it actually often opens the door to implants, according to Dr. Hedén, who says that 30 percent of his patients who first try injections eventually undergo breast augmentation. It may because the injections allow a woman to see how happy she is with larger breasts before deciding to undergo surgery.

In addition to a lack of long-term data, the gel has a few other drawbacks. The absorption of the material isn’t predicable, and the amount of gel that remains after a year varies anywhere from 5 to 85 percent. In addition, patients will probably need a touch-up after nine to 12 months, according to Dr. Hedén.

Since Macrolane does not have FDA approval, breast augmentation is the best procedure for women looking to enlarge their breasts. One other possible alternative is fat grafting. Used mainly for the face, some cosmetic surgeons now use the technique for breast enhancement as well.

Minimal-Scar Breast Implant Incisions Avoid the Breast

While there will always be scarring with breast implants, New You Magazine recently gave an overview of some of the incision techniques being used to reduce the visibility of scarring.

Minimal-Scar Breast Augmentation Incision Techniques

Dr. Angelo Cuzalina, an Oklahoma cosmetic surgeon, pointed out that: “Different patients have different definitions of ‘minimal-scar’ surgery. But for some women, a scar on the breast just isn’t acceptable.”

Here are some options for breast implant incisions that avoid the breast entirely.

Arm Pit: This technique uses an incision in the pocket of the armpit and works best for those with deep armpits. One of the advantages of this incision, according to Dr. Cuzalina, is that the surgeon can visualize the muscle with a camera during surgery.

Belly Button: The transumbilical breast augmentation (TUBA) has the incision through the belly button, so the incisions are hidden in the navel. One drawback is that only saline implants can be used, and Dr. Cuzalina said that patients without a lot of fat on their stomachs may not be the best candidates.

Tummy Tuck: The transabdominoplasty, or TABA technique, can be used when the patient is having a tummy tuck at the same time as the breast augmentation. The surgeon then uses the tummy tuck incision to place the implants. However, this technique can’t be used if the patient also wants a breast lift.

If you are considering an incision technique away from the breast, be sure that your cosmetic surgeon is experienced with the technique you want, as the minimal-scar techniques can require greater skill.

FDA Approves New Silicone Breast Implant

breast implant choices There is a new breast implant choice with the FDA’s approval of Sientra Inc.’s silicone gel breast implant.

Sientra’s implants have been approved for breast augmentation in women 22 years and older and for breast reconstruction in women of any age.

With the latest approval, there are now three silicone gel-filled breast implants approved by the FDA in the U.S. They are manufactured by Allergan, Mentor and Sientra.

“Sientra has successfully broken the existing duopoly in the U.S. by offering surgeons and patients a new choice,” said founder and CEO Hani Zeini, in a statement.

Zeini also said that the company will offer shaped implants in addition to standard round implants.

The approval was based on a three-year clinical trial of 1,788 participants. The trial had similar complications and outcomes found in studies of other breast implants.

“Data on these and other approved silicone gel-filled breast implants continue to demonstrate a reasonable assurance of safety and effectiveness,” said William Maisel, MD, deputy director for science in the FDA’s Center for Devices and Radiological Health.

Maisel stressed that women should be aware that breast implants are not lifetime devices and recognize that long-term monitoring is essential.

Post-approval, Sientra will continue to monitor the clinical trial participants for seven years. In addition, they will have a 10-year study of 4,782 women who receive Sientra silicone implants to collect data on long-term complications and outcomes. Finally, Sientra must also conduct case studies to evaluate the association between their implants and five rare diseases, including brain cancer, cervical/vulvar cancer and lymphoma.
